Back to school means it's time to complete the Student Verification Process. Every year parents/guardians must complete all forms in the Student Verification Process for each student enrolled in the District. The process is done through forms in
MASD’s PowerSchool Parent Portal.
Student Forms
Please complete all forms and agreements in the Parent Portal. It is especially important that families update forms relating to student emergency contact information and household information, including:
Form C1, School Communications-School Messenger, is new this year. This form collects information for contacting parents/guardians by phone, text, and email about District announcements, emergency messages, and weather-related messages. All changes for School Messenger will be made using this form now, rather than the need to log into School Messenger separately.
Form R1, Student Information Update, is also important. Please list all students under the age of 19 (regardless of whether they attend MASD or not) living in your home. Please also review and make any updates to your student's existing parent/guardian and emergency contact information, and ensure that all phone numbers are correct and up to date. Use the check boxes to mark if a contact has custody, lives with the child, can pick them up from school, or if they are simply an emergency contact.
