In preparation for the new school year, parents are encouraged to deliver medications and all required medical documentation to their child’s school prior to the first day of school. In accordance with Middletown Area School District’s policy, no prescription medication or over the counter medication (Tylenol, Ibuprofen, etc) will be administered by school personnel without a parent’s/guardian’s signature and specific written instructions from a physician.
MEDICATION DROP-OFF DATES
Tuesday, August 13
Middle School Medication Drop-Off, Middletown Area Middle School, 5-6:30 p.m.
Wednesday, August 14
Elementary Medication Drop-Off, all elementary buildings, 3 - 5 p.m.
High School Medication Drop-Off, Middletown Area High School, 3 - 5 p.m
MEDICATIONS AND REQUIRED PAPERWORK NOT RECEIVED AT DROP-OFF MAY PREVENT YOUR CHILD FROM RECEIVING THEIR MEDICATION(S) ON THE 1ST DAY OF SCHOOL.
Updated immunization information may also be brought to the school during this time.
WHAT TO BRING:
- Completed Student Medication Form
- Written physician’s order for EACH medication, including over the counter medications
- All medications in original labeled container
- Proof of immunizations (if applicable)
