Student Registration

Please note: In an effort to reduce costs, the Middletown Area School District has closed two elementary buildings, John C. Kunkel Elementary and Lyall J. Fink Elementary, for the summer months. They will reopen on August 16, 2011. Student registrations for Kunkel Elementary will be handled at Reid Elementary, 948-3360 and student registrations for Fink Elementary will be handled at the Middle School, 930-0739 until August 16, 2011. Registration forms may be picked up at Reid, the Middle School and High School from 8:00 am – 3:00 pm.

Only the parent/legal guardian may register a student, and must come into the office in person to complete the registration process.
Having all your documentation and forms completed prior to your appointment will expedite the enrollment process. Items required at the time of your appointment to register your child:

  • Completed district registration forms
  • Official Birth Certificate
  • Proof of Residency:
    • ONE (1) of the proofs must be one (1) of the following:
      • A copy of the Deed
      • Real Estate Tax Bill  or receipt
      • Agreement of Sale
      • Signed Lease
    • TWO (2) other proofs can be any of the following:
      • Valid PA Department of Transportation License or Identification
      • W-2 statement/IRS statement or tax return
      • Auto/Homeowners/Renters Insurance statement
      • Copy of State/Federal Program Enrollment
      • Voter registration card noting township or borough
      • A current utility bill (gas, electric, oil, water, or sewer)

Parent/legal guardian Photo Identification (Drivers License, non-driver identification, passport, military identification)

  • EVIDENCE OF IMMUNIZATION – Immunization records may be obtained from the school you are leaving or from your child’s physician.
  • Report Card/Transfer Card/Transcript
  • Name and address of previous school
  • If appropriate: most recent IEP, ER, RR, Agency Letter of Placement, or Court Order (Proof of Guardianship)
