Medication Drop-off Night – August 21

In preparation for the new school year, MASD will be hosting a Medication Drop-off Night to encourage parents to deliver medications and all required medical documentation to their child’s school prior to the first day of school. In accordance with Middletown Area School District’s policy, no prescription medication or over the counter medication (Tylenol, Ibuprofen, etc) will be administered by school personnel without a parent’s/guardian’s signature and specific written instructions from a physician.

MEDICATIONS AND REQUIRED PAPERWORK NOT RECEIVED ON DROP-OFF NIGHT MAY PREVENT YOUR CHILD FROM RECEIVING THEIR MEDICATION(S) ON THE 1ST DAY OF SCHOOL.

Updated immunization information for students who have been notified of potential exclusion from school may also be brought to the school during this time.

WHAT TO BRING: 

  • Completed Student Medication Form (located on raiderweb under “For Parents”)
  • Written physician’s order for EACH medication, including over the counter medications
  • All medications in original labeled container
  • Proof of immunizations (if applicable)
